To join Sequim Medical Associates we require a complete application packet to be filled out and returned for review by the MD you would like to see. Once the review process has happened ( typically within a week ) and your application has been accepted, we will call you and schedule you for the first available appointment. Please remember to bring your insurance cards, healthcare directives and any other paperwork you feel we should have on file for you at your first visit. For your convenience we have included links for all the forms below ( complete enrollment packet) or the individual pages. Note on the Trust Agreement, this form only needs to be filled out if you plan on pre-paying for the concierge fee quarterly, bi-annually or annually.
